These options may provide you with the help you need more quickly.

How to request an appointment

You can request an appointment:

  • using an online form
  • by phoning the surgery (please see ‘Request an appointment using an online form’ for details of what types of appointments can be booked over the phone). 
  • or in person (please see ’Request an appointment using an online form’ for details of what types of appointments can be booked in person).

Please note that for patients aged 16 and over, all requests for a GP or Physician Assistant appointment must go via our online form. If you are unable to complete the form yourself, and have no one to assist you, then a member of our reception team can help you to complete the form. This can be done over the phone or in person.

Request an appointment using an online form

You can tell us about your condition or symptoms using our "Rapid Health" online form. If you need an appointment, you will be able to choose an appointment time online.

All patients using this form need to have an email address linked to their medical records.

This form can only be used for patients aged 16 years and above. You need to be registered with the practice to use this service. When completing the form, you should use the email address that is on your medical records.

Information:

The online form is available from 07:30am to 18:00pm Monday to Friday, but may close earlier if the practice reaches its safe appointment capacity. If closed, it will reopen at 07:30am the next working day. Please consider alternatives like the pharmacy, Urgent Care Centre, 111, or A&E. You can still submit admin queries during this time but you cannot request appointments.

Please Note: For the following types of appointments, Rapid Health is not used. To book these, please contact the Practice directly by phone or in person:

  • Home visits
  • Appointments requiring an interpreter
  • Nurse appointments (e.g., blood tests, cervical screening, vaccinations, injections)
  • Annual health checks (e.g., for diabetes, high blood pressure, respiratory or heart conditions)
  • Medication reviews with a clinical pharmacist
  • Post-natal check-ups and 6-week baby checks
  • Contraceptive advice and prescriptions
  • NHS Health Checks
  • Follow-up appointments

Help during your appointment

When you request an appointment, please tell us if you need specific help, for example:

  • if you cannot come to the surgery
  • if you need an interpreter
  • or if you have any other access or communication needs

Cancel an appointment

If you cannot come to your appointment, please cancel it as soon as possible so that we can help another patient. You can cancel an appointment:

Urgent advice: Call 111 if you need urgent advice

NHS 111 can help if you have an urgent medical problem and you’re not sure what to do. To get help:

  • go to 111.nhs.uk (for people aged 5 and over only)
  • call 111

Available 24 hours a day, 7 days a week.

Immediate action required: Call 999 or go to A&E in an emergency

Medical emergencies can include:

  • loss of consciousness
  • an acute confused state
  • fits that are not stopping
  • chest pain
  • breathing difficulties
  • severe bleeding that cannot be stopped
  • severe allergic reactions
  • severe burns or scalds
